Abstract

Purpose
Using ARFI to examine peripheral pulmonary consolidation, in combination to B-mode ultrasound and Doppler for assesment the ability of ARFI technique in the differential diagnosis of those diseases.

Materials and methods
A total of 28 patients at the Medic Medical Center from October 2008 to December 2012, including 21 male patients, age from 18 to 79. Lobe pneumonia in 16 cases (57.1%), atelectasis in 6 cases (21.4%), bronchial cancer in 4 cases (14.2%), pulmonary lymphoma metastase 2 cases (7.1%). 6 cases underwent ultrasound elastic ARFI, including 3 cases lobe pneumonia and 3 cases of atelectasis. Each case was measured ARFI (VTQ) in 5 times. All cases were taken chest X-ray and blood tests. MDCT was used in cases of unclear diagnosis (10 cases, 35.7%). Medcalc statistical software was used to compare ARFI values (V=m/sec) between groups of patients.

Results
9 cases of lobe pneumonia are usually in lower lobe of the lung (87.5%), triangular appearance (94%), regular border (68.7%), homoechogenic with the liver pattern (25%), air bronchogram (94%), vascular pattern (9 cases, 56%), spectral Doppler 3-phase high resistance (7/9 cases, 77.8%). 2 cases of atelectasis are usually triangular appearance (100%), regular border (83.3%), hyperechoic (100%), vascular pattern (2 cases, 33.3%), spectral Doppler 3-phase high resistance (2/2 cases, 100%), air bronchogram (50%). 4 cases of bronchial cancer are usually oval appearance (75%), unregular border (75%), hypoechoic (100%), not air bronchogram, single vessel (4 cases, 100%), common arterial 1 phase low resistance (3/4 cases, 75%). Lymphoma is round appearance, echo poor look like a cyst and single vessel, no air bronchogram. The shear wave velocity means of lobe pneumonia range from 2.06 to 4.02 m/sec (mean=3.11±0.99 m/sec), and atelectasis range from 0.94 to 1.93 m/sec (mean=1.52±0.46 m/sec). There are significant statistically differences between the two groups with t=2.968 (p=0.034).

Conclusions
This is a preliminary study of ultrasound elasticity ARFI in the differential diagnosis of peripheral pulmonary consolidation, combined with B-mode and Doppler ultrasound. Further multicenter researchs with larger numbers are necessary to confirm the ability of this technique and application in clinical practice.

2/ Ultrasound in revelation chondral rib fracture and  bony rib fracture at an outpatient  clinic : A Vietnamese experience

Abstract:



OBJECTIVE:



To disclose chondral or bony rib fracture by ultrasound which are negative on X-ray film of minor blunt chest trauma patients.



METHODS:


A total of 50 patients suffering from minor blunt chest trauma without evidence of a rib fracture on chest X-ray film, were examined with a 9L4 MHz or 7.5 MHz linear transducer of ultrasound system (Siemens, Aloka, Medison). Statistical analysis was done to outline the ultrasound findings of these rib fractures.

RESULTS:


There were 50 patients showed chondral and bony rib lesions, whereas these 50 patients (100.0%) had no evidence of rib lesions on chest X-ray film. Fracture of the rib with a disruption of continuity of bony cortex near chrondral and bony rib junction was the most common finding in  45 (90%) patients. Chondral rib fractures were in five (10%) patients. Chondral rib fracture appeared as disruption of cortex, small echogenic lines in chondral rib, and bruised chondral rib was a small deformation of chondral cortex and echogenic area at trauma site which was painful site. Bony rib fractures significantly occurred in trauma patients, and the duration of pain in patients with chondral rib fractures was significantly longer than that of patients with bony rib fractures.

CONCLUSIONS:


Ultrasonography is a useful imaging method in disclosing the rib fractures (chondral and bony rib fractures) which were negative on chest X-ray film in minor blunt chest trauma. However, chondral rib fractures significantly occur less than bony rib fractures and result in a longer duration of pain.
